4 Types of PCOS

Many women with PCOS come to me saying they felt totally overwhelmed and confused and with all the (mis)information out there and had no idea where to start. One of the first steps to gaining clarity and control over your PCOS is in finding and addressing the underlying causes of your symptoms.

 

UNDERSTANDING PCOS

Polycystic Ovarian Syndrome (PCOS) is one of the most common hormonal conditions affecting 6-21% of women of reproductive age worldwide (so girl, you are not alone!). PCOS can affect hormones, fertility, skin, cardiovascular health and metabolism. That's why PCOS is called a syndrome, rather than a disease, as each woman presents a little differently. And why we now understand there are four different "phenotypes" of PCOS. Having PCOS increases your risk of:

  • insulin resistance
  • type-2 diabetes and gestational diabetes (in pregnancy)
  • fatty liver
  • high cholesterol
  • cardiovascular disease

The good news is that we can reduce these risks and potentially reverse the symptoms of PCOS through nutrition and lifestyle, and live happy healthy lives! To be diagnosed with PCOS, you need to have at least any 2 of the following 3 criteria:

  • Delayed ovulation or menstrual cycles (i.e. irregular periods or menstrual cycles 35 days or longer)
  • Hyperandrogenism (i.e. high levels of androgens - DHEAS, testosterone, androstenedione)
  • Polycystic ovaries on ultrasound

Did you know there are 4 main "types" of PCOS and that every woman with PCOS experiences it differently?

 

HERE IS A QUICK OVERVIEW OF THE 4 PCOS TYPES (YOU CAN BE 1 OR A MIX OF THESE TYPES OF PCOS):
1. Insulin Resistant PCOS
High insulin and leptin (hunger hormone) impair ovulation and tell the ovaries to produce more testosterone. Your symptoms may include high insulin and/or blood glucose levels, weight gain or difficulty losing weight, fatigue, sugar and carb cravings, feel irritable or shaky if you don't eat regularly. Balancing blood sugar levels is key with this type of PCOS.
 
2. Inflammatory PCOS
Inflammation (caused by stress, environmental toxins, gut dysfunction and inflammatory foods) in the body impairs ovulation and disrupts hormonal signalling causing high androgens. Your symptoms may include digestive issues, fatigue, headaches, joint pain, skin conditions and/or acne, and food intolerances. Reducing inflammation is key with this PCOS type.
 
3. Adrenal PCOS
This is driven by a highly sensitive response to stressors (physical, environmental, emotional) causing the adrenal glands to produce DHEAs, another type of androgen that impairs ovulation and disrupts hormonal balance. You may feel easily stressed, anxious, depressed, feel fatigued and/or "tired but wired", have difficulty recovering from exercise, have weight gain around your middle and find your menstrual cycle changes when you are more stressed. Nutrition and lifestyle modifications to reduce your body stress response is key for this type of PCOS.
 
4. Post pill PCOS
This can occur after stopping the oral contraceptive pill (OCP), which causes temporary high androgens. Like with other types of PCOS, high androgens impair ovulation causing the symptoms of PCOS. Your body is trying to re-balance its natural hormones, however is also dealing with nutrient depletion caused by the pill. Nutrition plays a big role in helping your body through this phase.
